Executive Director (ED) is responsible for overall leadership, direction, and clinical quality of the Agency. They oversee qualified clinical personnel, provision of home health services, and compliance with federal and state regulations. The Executive Director establishes and evaluates goals and objectives for agency services, promoting quality and organizational philosophy.
Asst Director will cultivate a culture of continuous improvement, facilitating Quality Skills Enhancement, Occupational Training & Curriculum Development programs. Responsibilities include cataloging and vetting Training and Employment Fund (TEF) curricula, overseeing staff, monitoring budgets, engaging stakeholders, and ensuring high-quality education services.
QDDP Will participate as a full member of the individual’s support network; develops an implementation strategy based upon the Personal Plan which outlines what is important to and for the participant; ensures supports and activities identified in the strategy are implemented; routinely reviews the participant’s strategy making adjustments as needed. Completes all required documentation in a timely manner to ensure compliance with the program’s rules and regulations. As well as:
Ensure that participants are provided the opportunity to engage in the community, developing relationships necessary for employment, volunteerism, development of personal interests and social experiences.
Reports on participant progress regarding outcomes set forth in the implementation strategy and ensures the outcomes continue to meet the needs and choices of the participant. Progress is reported to the participant and their support network.
Establish constructive working relationships with the participants and their support network in order to ensure the needs and desires of the participants are being met.
Protect the financial interests of participants by monitoring all money management transactions in checking and savings accounts and entitlements such as Social Security and Illinois assistance programs.
Ensure agency’s strategic plan, policies and philosophy are communicated and understood by participants and staff.
Aides in the development of the agency through promoting volunteerism, involvement in promotional activities, and providing education and awareness to the general public in order for Community Link to be a leader in programs and services.
Registered Nurse provides nursing services to seriously mentally ill consumers in a residential setting. Candidate must have a current and unrestricted Registered Nursing license valid in the State of Alabama, with preference given to those with experience working with seriously mentally ill clients.
Direct Care Professionals (DCP) are responsible for many things, including medication administration, personal cares, cooking, cleaning, and being a companion to our clients while they are out in the community. This position has a lot of opportunity for growth since every day can be a learning experience! House Manager will schedule and direct the DCP’s. They instruct and assist participants in activities of daily life to ensure learning, skill development and the facilitation of the greatest independence possible in accordance with the agency’s mission.
They must embrace the agency’s vision, mission, and core values. Additionally, the House Manager/ needs to be willing to engage in a person-centered philosophy and collaborative approach. They must demonstrate effective communication skills, flexibility to changing program needs, and display professionalism when engaging with staff, peers, supervisors, participants, and families. The House Manager must have genuine interest in the welfare of participants and their families.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Responsible for the supervision of the direct care staff. Carries out supervisory responsibilities in accordance with the agency’s policies and applicable laws. Responsibilities include scheduling, interviewing, hiring, training, and coaching employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
- Establishes the staffing pattern at the home to ensure participant needs and choices are supported. Secures substitute staff and/or personally provides shift coverage as needed.
- Be knowledgeable of each participant’s medical needs. Provide medical support in the event of an emergency.
- Plan, organize and direct activities for the overall operations of the homes to ensure participants reside in a safe, comfortable, sufficiently supplied home that enhances inclusion in their community and meets all regulatory compliances of the program.
- Provides oversight, modeling and training of the staff to ensure appropriate interactions, learning techniques, financial responsibility and basic care are performed in a manner of respect and dignity and that also meets the preferences and needs of the participants.
- Monitors and/or assists in administration of medications for participants according to rule.
- Ensures participants have opportunities to build relationships within their community, are accessing community resources and attending community events based upon their individual preferences and choices.
- Ensure all required and requested documentation is accurate and up to date.
